Right to Access

You have the right to inspect and obtain a copy of your health information maintained by Smooth Aesthetics Clinic. To request access, submit a written request to our Privacy Officer at appointments@smoothaesthetics.com.

You have the right to request that we amend your health information if you believe it is inaccurate or incomplete. Submit amendment requests in writing to our Privacy Officer.
